New Programs Offered By Independent Living, Inc.

Self-Directed Care Services

Through funding made possible by the New York State Office of Mental Health, Self-Directed Care (SDC) is a statewide program that provides funding to individuals that are HARP (Health And Recovery Plan) enrolled and HCBS eligible  (Home and Community Based Services) and are ready to take control of their lives and  take steps toward recovery.  Funds can be utilized to purchase items and or services that individuals feel can benefit them as they work towards their recovery.  The program can allow an individual to explore opportunities once thought not to be possible. Individuals work with a Support Broker to orient them into the program and to assist them in developing a budget for the allowable goods and services needed to reach the identified goals.

In order to be eligible for this program, participants must meet all three of the following eligibility criteria:

  • Individuals must be able to make decisions about the services on their own or with help,
  • Individuals must be enrolled in a Health and Recovery Plan (HARP)
    or an HIV Special Needs Plan
    (SNP), and
  • Individuals must be eligible to receive Behavioral Health Home and Community Based Services (HCBS).

NY Connects

Independent Living, Inc. is working in partnership with Westchester Independent Living Center on a new statewide program called NY Connects.  This is a locally based “No Wrong Door” that the system that provides one stop access to free, objective, comprehensive information and assistance for people of all ages needing long term services and supports. The program links individuals of all ages to long term services and supports regardless of payment source; whether it is private pay, public or a combination of both.

NY Connects has the ability to work with anyone who needs information, assistance or to explore their options regarding long term services and supports children or adults with disabilities, older adults, family members and caregivers, friends, or neighbors, veterans, and helping professionals.  Independent Living, Inc. is providing these services in Orange, Sullivan and Dutchess Counties.
