What is "Stay @ Home": The Nursing Facility Transition Program?

"Stay @ Home" is a Nursing Facility  Transition Program, operating in Orange County, NY by Independent Living, Inc.

Are you or someone you love  currently in or about to enter a Hospital, Rehab or Nursing Home? Before you go, know that you have rights and choices.

Independent Living's "Stay @ Home" Nursing Facility  Transition Program works with area  nursing facilities,   hospitals,  adult homes, assisted living and rehabilitation centers, Home health agencies, or any other community resources as well as residents to identify and establish the supports they need to transition to community settings of their own choosing.

It has been demonstrated that when given a choice people will select community living settings over nursing home placements. It is this Program's objective to provide those choices.

Utilizing the Olmstead Decision*, this Program identifies, assesses, plans, and supports the transition of nursing home residents, who are eager to and capable of returning to community living, back to integrated community living settings of their own choosing.

This Program enhances the quality of life for individuals by enabling them to 1) live independently, 2) come back to their families, 3) make decisions for themselves in the maximum extent possible, and 4) be part of their community

*IT’S THE LAW! Legislation was signed in the spirit of the U.S.  Supreme Court's 1999 Olmstead  decision, which empowers individuals with disabilities to live more independently. It enables qualified individuals with disabilities to receive the services they need in the most integrated setting appropriate for their overall needs

Utilizing Person Centered Planning methods, Independent Living works with residents to locate appropriate residential settings and establish the supports they will need to successfully continue in these settings, such as:

  • coordinating services,

  • assisting with benefits (i.e., SSI, SSDI, Pension, Worker's Compensation),

  • locating accessible housing in Orange County,

  • locating reliable contractors that specialize in home modifications,

  • accessing affordable medication, etc.

What community-based services are needed to make a successful transition out of a nursing home?

In order for individuals to successfully transition out of a nursing home, access to a range of available community-based services is required. Examples of these services are as follows:

  • Personal Care & Chore Services

  • Homemaker Services

  • Adult Day Care

  • Transportation

  • Personal Emergency Response Systems

  • Counseling

  • Environmental Modification

  • Private Duty Nursing

  • Medical Supplies/Equipment

  • Home Delivered Meals

  • Socialization

  • Independent Living Skills Training

  • Housing Subsidies

By organizing a variety of available services, seniors and people with disabilities can live safely in the community and enjoy greater personal independence.


The Nursing Facility Diversion Program

What is the Nursing Facility Diversion Program?

Independent Living assists seniors and people with all disabilities in Orange County avoid unwanted nursing home placement or to help get individuals discharged from facilities.

Independent Living provides unbiased information about available long-term care services and supports, assists people in obtaining needed available services, and advocates to assure that people receive the available services they need to live independently and safely in the community.

What can Independent Living provide?

Information -
Independent Living provides information about individual rights, community services, equipment, accessibility, and much more. Independent Living understands the complex long-term care system and can give you the information you need to make informed decisions about long-term care.

Assistance -
Independent Living staff can help you to determine what available services and supports are the right ones to meet your needs. For example, you might be saying, "I just need help getting out of bed in the morning" or "I need someone to help me with my housework." We will let you know what services are available so that you can choose which service will best meet your needs. If you want, we can assist you in obtaining the service.

Advocacy -
IL staff assists people in getting the benefits and services they are entitled to. We teach people to be self advocates who know their rights. We advocate for people to return home with the available community support services they need.

What IL believes
IL firmly believes that you should have the right to choose what services you want and where to receive them.

IL is excited about working with seniors and people with disabilities in Orange County to provide the information, assistance and advocacy that are needed to avoid unwanted nursing home placement.

What community-based services are needed to avoid a nursing facility placement ?

Health Care Proxy
The New York Health Care Proxy Law allows you to appoint someone you trust - for example, a family member or close friend - to make health care decisions for you if you lose the ability to make decisions yourself. By appointing a health care agent, you can make sure that health care providers follow your wishes.

  • What help will I have?

Independent Living can make sure you receive the help/services that meet your needs. In addition, Independent Living can introduce you to a new type of home care service which provides assistance with daily activities called Consumer Directed Personal Assistance Services (CDPAS)- Our "Options" program is an alternative to traditional home care programs. This program offers more flexibility and independence. It enables self-directing Medicaid recipients and/or their family members to hire, train, and supervise personal assistants.

  • How will I pay for it?

If you are currently living in a nursing home, your Social Security, SSI, or SSDI check goes to the nursing home each month and you receive about $55 of it to spend. When you live on your own, you receive your full check to pay for living expenses. Other sources, such as Medicaid, Worker's Comp, etc. will pay for some services. Independent Living will answer all your questions about financing your services.

  • Why Independent Living?

Independent Living can make sure you receive the help/services that meet your needs.   • What is the catch?

There is no catch and no fee for Independent Living's services. Independent Living receives funding that supports Community Reintegration Services. These services allow us to help you find the housing, equipment, and services you need to live in the community. And, Consumer Directed Personal Assistance Services are paid for by New York State's Medicaid program.

  • What is the first step?

If you are interested in returning to living in a community setting or would like more information, please contact us. We can set up an appointment to meet with you at your nursing home or at our office, wherever you feel more comfortable.
