What is the Nursing
Facility Diversion Program?
Independent Living assists seniors and people with
all disabilities in Orange County avoid unwanted nursing home placement
or to help get individuals discharged from facilities.
Independent Living provides
unbiased information about available long-term care services and
supports, assists people in obtaining needed available services, and
advocates to assure that people receive the available services they need
to live independently and safely in the community.
What can Independent Living provide?
Information -
Independent Living provides information about individual rights,
community services, equipment, accessibility, and much more. Independent
Living understands the complex long-term care system and can give you
the information you need to make informed decisions about long-term

Assistance -
Independent Living staff can help you to determine what available
services and supports are the right ones to meet your needs. For
example, you might be saying, "I just need help getting out of bed in
the morning" or "I need someone to help me with my housework." We will
let you know what services are available so that you can choose which
service will best meet your needs. If you want, we can assist you in
obtaining the service.
Advocacy -
IL staff assists people in getting the benefits and services they are
entitled to. We teach people to be self advocates who know their rights.
We advocate for people to return home with the available community
support services they need.
What IL believes
IL firmly believes that you should have the right to choose what
services you want and where to receive them.
