A program that provides a unique alternative to traditional attendant care, giving individuals being served the opportunity to take control over the personal care services they receive - deciding HOW and WHEN and BY WHOM those services are provided. Provided in cooperation with the local Departments of Social Services in Orange, Ulster, Sullivan, and Dutchess Counties

MEDICAID SERVICE COORDINATION

 

(for individuals with Developmental Disabilities and Traumatic Brain Injury)

 

WHAT IS MEDICAID SERVICE COORDINATION (MSC)?
 

MSC is a Medicaid State Plan service provided by the Office of Mental Retardation and Developmental Disabilities (OMRDD) which assists persons with mental retardation and developmental disabilities and persons with Traumatic Brain Injury (TBI) in gaining access to necessary services and supports.

MSC is provided by qualified coordinators and uses a person centered planning process. MSC promotes individual choice, individualized services and supports, and consumer satisfaction.

At Independent Living, Inc we will assist people with developmental disabilities and persons with Traumatic Brain Injury (TBI) in gaining access to necessary services and supports.

WHO IS ELIGIBLE FOR MSC? A person must meet all of the following criteria:

  • Must be enrolled in Medicaid.

  • Must have a documented diagnosis of mental retardation or developmental disability.

  • Must demonstrate a need for ongoing and comprehensive service coordination.

  • Must choose to receive MSC.

  • Must not permanently live in an ICF, Developmental Center, Psychiatric Hospital, Small Residential Unit, Nursing Facility, Hospital or any other Medicaid funded institutional setting that provides service coordination.

  • Must not be enrolled in another comprehensive Medicaid long-term care Service Coordination (e.g. Long Term Home Health Care).

WHAT IS THE ROLE OF MEDICAID SERVICE COORDINATOR
The MSC helps explore what the person wants and needs in life, and then assists them in getting it. The activities of the MSC fall into three general categories:
• Individualized Service Planning; Advocating; Record Keeping

The Medicaid Service Coordinator facilitates assessment, linkage, and referral for the desired or needed service/support as it applies to each person. This is accomplished through coordination and service planning, advocacy, and monitoring of services.

EARLY CHILDHOOD DIRECTION CENTER (ECDC)

The EARLY CHILDHOOD DIRECTION CENTER (ECDC) at Independent Living, Inc. offers FREE, neutral information and referral services concerning children with disabilities:
  • Early Intervention Programs

  • Preschool Special Education

  • Child evaluation and assessment

  • Medical, Educational, and Social Services

  • Transportation

  • Day Care and Preschool Programs

  • Training Opportunities for Parents and Professionals

  • Other parent resources

  Other services include: 

  • matching the needs of children with programs and community RESOURCES
  • assisting parents in making fully informed decisions
  • Helping families to obtain the specific services they need
  • following up to ensure that children are Receiving appropriate support
  • coordinating services between agencies
  • providing parent education
  • training day care, nursery school, AND PrESCHOOL Providers
  • providing a resource library

Work Incentives Planning and Assistance (WIPA)

 

(Formerly) Benefits Advisement  Assistance and Outreach (BPA&O)

Are you wondering how working will impact you SSDI or SSI benefits...

Are you receiving SSDI or SSI benefits, working now, or thinking about returning to work? Independent Living, Inc. can assist you in preparing a work plan so that you can go to work, keep your benefits, and continue receiving Medicare or Medicaid. 

We serve many Hudson Valley counties (including Orange, Ulster, Dutchess). IL provides services aimed at strengthening the confidence of SSI and SSA beneficiaries in taking full advantage of available work incentives, and encouraging routine collaboration among community agencies that provide information and services to persons who may be interested in entering or re-entering the workforce.

We also provide information on the Medicaid Buy In and are work in close collaboration with the County Disability Program Navigator programs.

lOur commitment to persons with psychiatric disabilities began in 1987 when our doors first opened. Today, we offer a wide range of services many of which are provided by individuals who are themselves either current or former service recipients.

STUDENT SERVICES

we also offer an array of transition support services that assist students in crossing the bridge from school to adult life. The     program's staff engage students attending local schools in a whole life planning process that includes counseling and training to help clarify and organize personal interests and to address individual needs related to family, skills, behavior and community.

Providing weekend respite services and an after school latchkey program for students with developmental disabilities

Serving as a  bridge between the Deaf and hearing cultures in 4-counties, this program is dedicated to providing community outreach and education, advocacy, service coordination counseling and technical support.

"Stay @ Home" is a Nursing Facility  Transition Program, operating in Orange County, NY by Independent Living, Inc.

Question: Are you or someone you love  currently in or about to enter a Hospital, Rehab or Nursing Home? Before you go, know that you have rights and choices.

Independent Living's Nursing Facility "Stay @ Home" Transition Program works with area  nursing facilities,   hospitals,  adult homes, assisted living and rehabilitation centers, Home health agencies, or any other community resources as well as residents to identify and establish the supports they need to transition to community settings of their own choosing.

It has been demonstrated that when given a choice people will select community living settings over nursing home placements. It is this Program's objective to provide those choices.

Utilizing the Olmstead Decision*, this Program identifies, assesses, plans, and supports the transition of nursing home residents, who are eager to and capable of returning to community living, back to integrated community living settings of their own choosing.

This Program enhances the quality of life for individuals by enabling them to 1) live independently, 2) come back to their families, 3) make decisions for themselves in the maximum extent possible, and 4) be part of their community

*IT’S THE LAW! Legislation was signed in the spirit of the U.S.  Supreme Court's 1999 Olmstead  decision, which empowers individuals with disabilities to live more independently. It enables qualified individuals with disabilities to receive the services they need in the most integrated setting appropriate for their overall needs

Utilizing Person Centered Planning methods, Independent Living works with residents to locate appropriate residential settings and establish the supports they will need to successfully continue in these settings, such as:

  • coordinating services,
  • assisting with benefits (i.e., SSI, SSDI, Pension, Worker's Compensation),
  • locating accessible housing in Orange County,
  • locating reliable contractors that specialize in home modifications,
  • accessing affordable medication, etc.


  • These services - directed to local municipalities, commercial property owners, employers, businesses, schools, medical facilities, and other provider agencies - are intended to ensure and enforce compliance with the provisions of the Americans with Disabilities Act of 1990.

    Housing, Vocational & Homeless Supportive Services

    MENTAL HEALTH SERVICES  (111 Broadway)

    Case Management Services (for individuals living in Cornerstone Residence -supportive housing)   Offering a holistic, person-centered approach to facilitating connections with provider agencies and community amenities, as well as the attainment  of  personal goals.   Providing novel supports that help to ensure long-term residential stability, personal recovery, and  health and general well-being.

     

     Supportive Housing "Scattered Sites" (HUD) - HUD funded Supportive Housing Program.  Participants, who must be homeless and have a disability, participate with case manager in housing search and are then provided a rental subsidy and case management services.

     

    Vocational Case Management (OTDA) - We work with tenants at the Cornerstone residences to create an individualized employment and/or educational plan.  Vocational Case Manager will assist tenants with their needs, which may include resume writing, interview skills, job search, transportation assistance, and connections to local employers, VESID, OCCC and other services.

    After-Hours Drop-In Center   - Monday - Friday 5:30 - 7:30 at 111 Broadway - Provides a safe, Wellness focused after-hours environment dedicated to fostering healthy lifestyles, motivation, independence, self-direction, employment, social integration, community participation, and the ongoing provision of healthy natural supports for persons having psychiatric disabilities.
